Case Management

Case management helps clients and families navigate complex systems so care, resources, and daily needs work together rather than compete. This support is part of Evision Mental Health Services’ holistic, wraparound approach to community-based behavioral health care.

Case managers help identify priorities such as housing, benefits, medical care, school coordination, employment support, transportation, referrals, and communication between providers. Services are individualized based on each client’s goals, needs, treatment plan, and current level of support.

Case management may be paired with OMHC, PRP, IOP, medication management, residential support, or employment services to create a coordinated continuum of care. The goal is to reduce barriers, strengthen stability, and help clients remain meaningfully engaged at home, at work, at school, and in the community.
